3K: Tough Transitions: Optimizing Care for Young Adults through Pediatric to Adult Hospice and Palliative Care Transitions

Especially for young adult patients with complex medical needs, transitions from pediatric to adult care teams are often challenging. In this session, attendees will reflect on their own practices in transitions of care and study best practices for enabling smooth transitions for young adults.

By the end of the session, learners will be able to:

  • Identify key barriers for pediatric and adult providers in transitioning care in hospice and palliative care teams
  • Recognize effective strategies such as anticipatory guidance and resources such as transition readiness questionnaires which can be adapted for this patient population to better facilitate transitions of care for young adults with serious life-limiting illnesses
  • Develop an interdisciplinary framework and practice language through role-play and interactive small group activity to enable transitions of care
