4D: POLST, It's More Than an Order and a Form

The Provider Orders for Life Sustaining Treatment (POLST) conversation occurs in the advance care planning process.

POLST is a portable medical order that gives patients with serious illness options over treatment they do or do not want. POLTS doesn’t replace a health care directive.

By the end of the session, learners will be able to:

  • Describe how POLST fits in advance care planning
  • Identify key words and concepts in the POLST conversation
  • Create a strategy to include POLST conversations into goals of care planning discussions
