This session will reflect on current demographic trends and their impact in the care of diverse communities living with serious illness and at the end-of-life. We will review health inequalities in palliative and hospice care and discuss shifting our focus from cultural competence to cultural humility.

• Identify and challenge power imbalances in the care of diverse communities with advanced illness.
• Approach family participation through a family system’s lens.
• Identify tools for effective intercultural communication.
